InspireTherapy

For select patients with moderate to severe obstructive sleep apnea who haven’t tolerated CPAP, Inspire therapy may be an option. It’s a small implanted device that gently stimulates an airway nerve during sleep, helping keep the airway open without a mask or hose.

What Is Inspire Therapy?

Inspire is an FDA-approved upper airway stimulation system designed for adults with moderate to severe obstructive sleep apnea who haven’t been able to use CPAP. A small device is implanted under the skin of the chest, with a thin lead that connects to the hypoglossal nerve, which controls the tongue. While you sleep, the device delivers gentle electrical pulses that move the tongue forward and keep the airway open. You turn the system on at night with a small handheld remote and off in the morning.

Who's a Good Candidate?

Inspire therapy may be considered if you:

Have moderate to severe obstructive sleep apnea confirmed by a sleep study
Haven't been able to consistently use CPAP
Are within the body mass index range set by the device manufacturer
Don't have certain types of airway anatomy that aren't a fit (your provider will check)
Are 18 or older and meet other clinical criteria

It isn't right for everyone.

Inspire isn’t for everyone. Eligibility involves both your sleep study results and an airway exam your provider performs.

How Inspire Therapy Works

Treatment with Inspire involves a few steps:

An evaluation, including review of your sleep study and an exam of your airway

A short outpatient procedure to implant the device, usually under general anesthesia

A healing period before the system is activated

Activation and gradual adjustment of stimulation settings with your provider

Nightly use, turned on with a small remote before bed and off in the morning

Implantation is typically done through small incisions in the chest and under the jaw.

Recovery & Results

Most patients go home the same day as the implant procedure. Mild soreness, swelling, and bruising at the incisions are common for one to two weeks. The device is usually activated about a month after implantation, and stimulation settings are fine-tuned over several visits.

Many patients report better sleep and fewer apnea events once the device is dialed in, though individual results vary based on anatomy, settings, and consistent nightly use. A follow-up sleep study is typically used to confirm how well Inspire is treating your apnea.

Risks & Safety

Inspire involves a surgical implant, so there are risks to weigh. Possible risks include:

Pain, swelling, or bruising at the incision sites
Temporary tongue weakness or soreness
Infection
Device or lead issues that may require revision
Discomfort from stimulation that may need adjustment
Rare risks related to anesthesia or surgery

Your ENT provider will review the specific risks with you so you can make an informed decision.

How is Inspire different from CPAP?
CPAP uses external air pressure through a mask to keep the airway open. Inspire works from the inside by stimulating an airway nerve. There's no mask, hose, or machine on your nightstand. It's not better for everyone, though, and your provider can help compare.
Inspire has clinical criteria that include a body mass index range and other factors. Your provider will check whether you meet the current criteria based on your specific situation.
Most patients describe it as a gentle pulling or tingling that's manageable. The device is adjusted to a level that's effective for your apnea but tolerable for you.
The Inspire system is designed to be compatible with certain MRI scans under specific conditions. Always tell your imaging team you have an Inspire device so they can plan appropriately.
Yes. A virtual visit is a good place to review your sleep history and CPAP experience. Inspire requires an in-person airway exam, often with a scope, before it's considered.
